When reaches for container or unit dose, Compare label with MAR after retrieval from the rawer or immediately before pouring from a multi dose container, When replacing the container to the drawer or shelf or before giving the medication to the patient

Right med, Right patient, Right dose, Right route, Right time, Right reason, Right assessment, Right documentation, Right response, Right Education/ Right to refuse

Has pt receive med before?, Does this med make sense?, Why is this patient taking this med?

Check pts name and DOB (2 identifiers)

Correct dose, Is this a high or low dose?, Does administration of this med require math?

Is this administration route appropriate?, Can my pt swallow?, Does route need to be adjusted?

When was the last time this med was given?, What window are you allowed to give meds?

Does pts condition, symptoms, health status warrant taking this med?

Vitals?/ Labs?, Med admin parameters?, Assessment findings?

Document administration, Document according to facility policy

How is the pt responding to the med?, Is it effective?, Assess lab values as appropriate, Side effects/ Adverse effects

Why receiving, purpose, expected effects, SE?, Directions for taking, Keep pts level of knowledge in mind
